Nexus Family Healing is looking to add a Case Manager - Residential to join our team in Manteno, IL. If you have experience working with adolescent females facing mental health and behavioral challenges, this is your opportunity to make a meaningful impact!

Located south of Chicago, in the small, rural community of Manteno, IL. We offer a unique homelike setting, where boys and girls aged 12-21 live, eat, exercise, and attend school on campus. Helping youth face difficult challenges isn't easy, but for most of us, it's the most satisfying work we've ever done.

Schedule: Tuesday-Saturday

  • Tuesday: 9:00 AM - 5:00 PM
  • Saturday: 12:00 PM - 8:00 PM or 1:00 PM - 9:00 PM (rotating)

Some flexibility required to meet client needs.

Pay: $24-$26 per hour

Position Summary: Our Residential Case Manager plays a vital role in ensuring the unique treatment needs of each assigned youth are met. This position is key to the success of our program-and your support can be life-changing for the youth we serve.

Primary Responsibilities:

  • Facilitating/Co-facilitating case conferences and client-oriented team meetings with both internal and external resources
  • Actively participating in the development and implementation of individual treatment plans, compiling assessment materials, maintaining required clinical documentation, and reporting within provided deadlines.
  • Collaborating with family members, licensing representatives, parole officers, court officials, and other organizational parties to ensure ongoing consensus on therapeutic treatment direction, behavior expectations, progress, and outcomes.
  • Engaging in counseling sessions and therapeutic interventions within treatment parameters
  • Ensuring completion of all required documentation standards
  • Serving as a mentor and role model by demonstrating compassion and understanding for feelings, opinions and experience of others, and communicating and interacting in a manner that is honest, positive, respectful, and professional.
  • Protecting resident safety and participating with coworkers to maintain an environment that is positive, respectful, clean, and therapeutic.

Requirements

Required:

  • Bachelor's degree in a clinical discipline (Psychology, Social Work, etc.) required. In lieu of clinical degree, any bachelor's degree plus two years of experience in case management and/or social work.
  • Strong critical thinking, organization, and communication skills, along with attention to detail and the ability to exercise discretion with highly sensitive and confidential information.
  • Must be MHP eligible.
  • Minimum 21 years of age.
  • Valid driver's license required (must meet state driving criteria and pass corporate eligibility standards).
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.

Preferred:

  • Bachelor's degree in a clinical discipline (Psychology, Social Work, etc.) and 3+ years of experience in case management and/or Social Work.
  • Experience working with adolescents facing mental health and behavioral challenges, particularly survivors of human trafficking and sexual abuse preferred.
