Description
Provide goal directed activities, which will assist the client by locating, coordinating, making linkages and monitoring necessary treatment and services available to the client. The Care Coordinator activities are community based and are delivered either in the clients environment, in community agencies or in our agency. Service will be accessible to the client and their families in the home, school, workplace or wherever needed.
DUTIES & RESPONSIBILITIES:
Direct Services
- In accordance with treatment plan, goals, objectives and strategies, the Care Coordinator will monitor and assist the client with developing and maintaining sufficiently competent skills to live successfully in the community, assisting supervising and prompting will be provided as necessary.
- Care Coordinators will assist and support the clients with activities of daily living or other billable services by providing transportation.
- Provide support to client in effort to maintaining a substance free life style.
- Complete progress notes for each treatment activity in accordance with agencys procedures and standards. Maintain 100% productivity at all times.
- Participates and contributes to case staffing and treatment team meetings.
- Function as a liaison and advocate for clients with community resources. Assist clients with completing and following through with entitlement applications (Medicaid, Medicare, social security issues, food stamps, Presumptive Eligibility, Behavioral Primary HealthCare Coordination BPHC etc.). Care Coordinators will provide aggressive outreach, after hours care and aftercare/follow-up services including group sessions.
- The Care Coordinators will be aware of and observe symptoms of possible relapse and promptly report it to the appropriate clinician/counselor. Also, when needed will assist the clients with following through with interventions to circumvent relapse.
- Represent Edgewater Systems within the community. Educate the community as well as family members, staff and others regarding the services provided by Edgewater Behavioral Primary Care.
- If in clients IICP (Individualized Integrated Care Plan) for drug/ alcohol screening the care coordinator will refer the client to the appropriate identified provider.
- Utilize literature, workshops and other training aids to continually improve clinical and administrative skills.
- Treat all clients with dignity, courtesy and respect, recognizing their right to have requests, needs and complaints responded to in a professional and timely manner.
- Work in collaboration with Clinician in defining treatment plans and provision of necessary services to clients.
- Participate in Community Outreach events.
- Perform other job-related duties as assigned.
Administrative
- Provide administrative documentation in a timely manner.
- Follow-up with clients (or refer to Engagement Specialist for follow-up) regarding missed or cancelled appointments via phone calls and/or letters.
GENERAL
- Conduct all professional activity in an ethical manner.
- Conform to all applicable policies and procedures.
- In conjunction with other staff assist in identifying and eliminating any situation which
- jeopardizes the health and/or safety of clients, staff, or others entering the community mental health center operations.
- Utilize literature, workshops and other training aids to continually improve case management skills.

ESSENTIAL JOB REQUIREMENTS:
- Ability to relate to individuals of all ages
- Must be able to communicate with clients frequently in a manner able way.
- Legible writing skills
- Ability to assess and provide for client needs; be supportive of clients.
- Must be able to climb stairs 50% of the time
- Have use of personal vehicle

Qualifications
POSITION REQUIREMENTS:
- Education Bachelors degree in in Psychology, Social Work or other related human service field
- Experience Minimum of 3-5-year experience as a case manager or social worker in mental health, social services or substance abuse preferred.
- Other - Possess a valid drivers license.
- Furnish own inspected, automobile as needed for Center work and transportation of clients.
- A good driving records.
- Appropriate automobile insurance and valid license tags.
- Maintain a vehicle free of mechanical problems and carry personal vehicle insurance.
