Role Overview:
The Associate Care Coordinator assists low risk Enrollees, appropriate for care coordination and care management services, in achieving their optimal level of health. The Associate Care Coordinator must have relevant experience and education to work telephonically and face to face with Enrollees to be able to complete surveys/assessments and update a plan of care for low risk members.
Responsibilities:
- This candidate will provide care coordination services to low risk/low acuity members
- Provides access to a single point of contact for all questions or inquiries
- Conducts surveys and assessments with Enrollees and/ or their caregiver
- Conducts periodic field assessments as required
- Create and update Individualized Care Plan
- Conducts medication review
- Provides periodic monitoring of health, functional and mental status
- Ensures the provision of services in the least restrictive setting and transition support across and between specialties and care settings
- Connects Enrollees to services that promote community living and help to delay or avoid nursing facility placement
- Coordinates with social service agencies (e.g., local departments of health, social services and community based organizations)
- May refer Enrollees to state, local and/or other community resources
Education:
- Bachelor's Degree in a health or social services related field
- Certification as a Case Manager or a Community Health Worker preferred
Licensure
- Valid drivers license with car insurance
Experience:
- 1 to 3 years social services and/or care management/coordination experience working with clients in a home/community setting, including those receiving physical health, behavioral health and social services preferred
- 3 to 5 years of Case Management experience preferred
- Experience in a remote/telephonic environment preferred
