Social Services Case Manager Jobs in Ontario

A Social Services Case Manager is a professional in the social work sector who plans and coordinates care for clients. They are responsible for assessing, monitoring, implementing, and evaluating actions required to meet the client's health and human services related needs. They act as a link between clients and the appropriate services and resources available from various social services agencies. Most employers require a bachelor's degree in social work, psychology, or a related field, although a master's degree may be preferred. Additional certifications such as the Certified Social Work Case Manager (C-SWCM) or Certified Advanced Social Work Case Manager (C-ASWCM) from the National Association of Social Workers (NASW) can enhance job prospects. Prior to becoming a Social Services Case Manager, a person may have roles such as a Case Worker, Social Worker, or Mental Health Counselor.
