They are responsible for assessing, monitoring, implementing, and evaluating actions required to meet the client's health and human services related needs. They act as a link between clients and the appropriate services and resources available from various social services agencies. This role requires a deep understanding of social systems, excellent communication skills, and the ability to work with diverse populations.
