Social Services Case Manager Jobs in California

A Social Services Case Manager is a professional in the social work sector who plans and coordinates care for clients. They are responsible for assessing, monitoring, implementing, and evaluating actions required to meet the client's health and human services related needs. They act as a link between clients and the appropriate services and resources available from various social services agencies.
