RN Case Manager

Key responsibilities of a hospital case manager: Patient assessment: Conduct comprehensive patient assessments to identify medical needs, social determinants of health, and discharge planning requirements. Care coordination: Collaborate with physicians, nurses, therapists, and other healthcare providers to develop and implement individualized treatment plans. Discharge planning: Create detailed discharge plans that include follow-up appointments, medication management, community resource referrals, and necessary home care arrangements. Utilization management: Monitor patient care to ensure appropriate resource utilization and cost-effectiveness. Insurance coordination: Assist UR nurses with communication with insurance companies to verify benefits, obtain prior authorizations, and address billing issues. Patient education: Provide patients and families with information regarding their medical condition, treatment options, and self-management strategies. Community outreach: Connect patients with necessary community services like transportation, housing assistance, and support groups. Documentation: Maintain detailed patient records and documentation regarding care coordination activities. Other misc: Strong communication and interpersonal skills to interact with patients, families, and healthcare team members. Excellent organizational and time management abilities. Knowledge of healthcare regulations, insurance policies, and community resources.
