SUMMARY:
To provide clinical supervision to social work team members and to maintain responsibility for the provision of social work services to support each residents independence and functioning in the facility through psychosocial assessment and intervention. Oversight and coordination of admissions to the facility and the admission process.
RESPONSIBILITIES:
- Assesses resident needs and determines appropriate mode of intervention. Possesses appropriate age specific knowledge as it relates to resident care and intervention. Conducts resident interviews and records appropriately in the MDS 3. and medical record as applicable. Formulates assessments and develops care plans in accordance with accepted social work policy and documents in the medical record. Involves resident/family in intervention planning and in the assessment process. Coordinates care plan meetings for assigned residents and documents outcomes and actions as they relate to the care plan meeting. Follows up at appropriate intervals as it relates to issues addressed in the care plan meeting.
- Coordinates discharge planning for assigned residents. Acts as intermediary between facility and external providers. Involves resident/family in discharge planning process.
- Provides management oversight for social workers. May coach, mentor, and train new staff in their assigned duties in order to achieve a standard of excellence in the department.
- Provides clinical supervision of social workers at Hill Haven at least one day per week and presents observations and clinical process improvement recommendations to that facilitys administration.
- Collaborates with the Mgr, Admissions LTC on all issues related to the admission and placement of residents.
- Coordinates admission process and provides oversight for the intake coordinator. Acts as liaison between facility and hospital care managers to provide for seamless transition of care.
- Maintains a modified social work case load.
- Participates as an active member of the management team and plans for program direction and program needs and provides social work input into these planning processes.
- Supervises social work staff in day to day activities and completes a yearly evaluation of staff performance through performance appraisals.
- Evaluates and identifies staffing pattern needs for the social work department including the coordination of vacation/time off schedules.
- Coordinates the recruitment/hiring/disciplinary action for social work staff with RGHS Human Resources.
- Develops, monitors and maintains accountability for the social work/admissions departmental budget.
REQUIRED QUALIFICATIONS:
- BSW required.
- 3 years experience or equivalent progressive experience including paid or unpaid internships in the field of Social Work required.
PREFERRED QUALIFICATIONS:
- Strong analytical, computer and communication skills
- Masters degree in Social Work preferred

$60,500.00 - $85,000.00CITY:

14513The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
