Our rehabilitation programs provide ongoing care and specialized treatment to patients throughout their recovery journey. We offer customized, individualized rehabilitation tailored to the specific needs of those recovering from a stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.

How you'll contribute

The Case Manager of Rehabilitation is responsible for providing care coordination including needs assessment and identification of care options, communication with patients and families in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

What we're looking for

  • Current licensure as a Registered Nurse, Social Worker, or licensed healthcare professional such as a Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist

  • Certification in Case Management or Rehabilitation Nursing preferred, such as:

    • Commission for Case Manager Certification (CCM)
    • Association of Rehabilitation Nurses (CRRN)
    • American Case Management Association (ACM)
    • Board Certification in Case Management by the ANCC (e.g., RN-BC)
  • Minimum of two (2) years of case management or social work experience in an inpatient setting highly preferred; acute care or rehabilitation hospital experience preferred

  • Basic Life Support (BLS) certification through the American Heart Association
