About Us*Fidelity Behavioral Health is dedicated to transforming the lives of justice-involved men with severe mental health impairments.Our mission is to provide a *holistic, person-centered approach* that addresses the mind, body, and spiritaempowering our clients to rediscover purpose, build resilience, and foster a genuine sense of belonging.We are equally committed to creating a *healthy, supportive, and balanced work culture* where our team members feel valued, heard, and equipped to thrive.*Position Overview*We are seeking a *compassionate, resourceful, and highly organized Behavioral Health Case Manager* to join our growing team.In this role, you will be the bridge between our clients and the services they need to succeed after completing our program.You will work directly with clientsamost of whom are unhousedato connect them with housing, vocational opportunities, healthcare, and other essential resources.Case Managers at Fidelity Behavioral Health work in-person at the client's place of residence.And meet with each client weekly to set goals, navigate challenges, and plan for successful reentry into the community.*Key Responsibilities** Meet regularly with clients to assess needs, set goals, and monitor progress.* Research, identify, and connect clients with community resources for housing, employment, medical and dental care, mental health services, financial aid, and other essential supports.* Assist clients in obtaining vital documents, EBT cards, SSI/SSDI, Medicaid, and other benefits.* Facilitate employment readiness through job training referrals and vocational resource connections.* Document all client interactions and services accurately and promptly in the EMR system.* Support clients from intake through discharge by identifying strengths, providing life skills education, and developing individualized discharge plans.* Engage with clients' families, employers, and community networks to strengthen support systems.* Collaborate closely with the clinical and healthcare team to ensure seamless integration of services and treatment plans.*Qualifications** *Education:* Bachelor's degree in a human services field preferred.* *Experience:* Strong knowledge of available social services and the processes required to access them.* *Skills & Abilities:** Proficient in online research for resources.* Extensive housing and vocational resource connections.* Knowledge of behavior management principles.* Strong written and verbal communication skills.* Ability to work collaboratively in a team environment.* Compassionate, non-judgmental approach toward individuals with mental health and substance use challenges.*Why Join Us?*At Fidelity Behavioral Health, your work matters.You'll be part of a mission-driven team helping clients break cycles, build stability, and create new futures.We offer a supportive environment where innovation, empathy, and collaboration are valued every day.Job Type: Full-timePay: $22.00 - $30.01 per hourExpected hours: 40 a 45 per weekBenefits:* 401(k)* Dental insurance* Employee assistance program* Flexible schedule* Health insurance* Paid time off* Professional development assistance* Retirement plan* Vision insurancePeople with a criminal record are encouraged to applyEducation:* Bachelor's (Preferred)Experience:* Case management: 1 year (Preferred)Ability to Relocate:* Westminster, CO 80031: Relocate before starting work (Required)Work Location: Hybrid remote in Westminster, CO 80031
