Program: Amarillo, Texas – Assertive Community Treatment (ACT) Team

Salary range:

Case Manager I: $40,809.60 Annually

Case Manager II: $43,305.60 Annually

Case Manager III: $46,009.60 Annually

Shift available: Full-time

Essential duties and responsibilities:

  • Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
  • Responsible for service delivery on the Assertive Community Treatment (ACT) team
  • This is an intensive adult caseload for individuals with behavioral health issues in addition to substance use, involvement in the criminal justice system, and/or frequent use of inpatient hospitalization using a team approach
  • Services include referral/linkage to community resources, case management, substance abuse treatment, housing and/or employment skills training
  • Will be trained in state-approved curriculum that emphasize evidence-based practices to help improve daily functioning stability in the community
  • Position will provide culturally competent care activities and collaboration among all participants involved with the individual’s care
  • Additional duties can include assessment and treatment planning with clients, working within the team to ensure appropriate services are provided, and engaging individuals by taking a person-centered approach to help identify goals for services/treatment
  • The position will assist & collaborate with team and supervisor to ensure evidence-based ACT model of treatment is provided
  • Works with all programs in Adult Behavioral Health Team in promote skills development and community stability for our client population
  • Adheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromised

Required qualifications to be successful in the job:

  • Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field for QMHP credentialing
  • For Case Manager II, minimum one (1) year of experience in behavioral health case management
  • For Case Manager III, minimum two (2) years of experience in behavioral health case management
  • Graduate work in a related field may be substituted for up to one (1) year of experience

  • Must have a valid Texas driver’s license and be insurable under agency vehicle policy
