JOB DESCRIPTION: The Trauma Case Manager (CM) serves a critical role in coordinating care for patients who have experienced acute physical trauma, guiding them through complex medical, psychosocial, and recovery pathways. Working across the continuum of carefrom emergency intervention to rehabilitation and dischargethe Trauma CM ensures timely, efficient, and patient-centered service delivery. They collaborate closely with interdisciplinary teams to optimize clinical outcomes, manage healthcare resources, and address barriers to recovery, including biopsychosocial challenges, housing instability, substance use, and mental health concerns. As a vital liaison among patients, families, providers, payers, and community resources, the Trauma CM facilitates seamless transitions of care and advocates for trauma-informed approaches that promote healing, resilience, and long-term well-being. EDUCATION: Master's in social work from an accredited school of social work. LICSW preferred. OR Current RN licensure or compact licensure recognized by the State of Vermont is required. OR Master's degree from an accredited institution in a Health or Human Services field. Licensure preferred. OR Bachelor's of Science or higher degree from an Accredited Occupational Therapy Program. Licensed or eligible to be licensed to practice occupational therapy in the State of Vermont. OR Bachelor's of Science or higher degree from an Accredited Physical Therapy program. Licensed or eligible to be licensed to practice physical therapy in the State of Vermont. EXPERIENCE: RN: Minimum of (3) years of clinical experience in a related field. SW: Minimum of (3) years experience in a clinical setting, including (2) years in a medical setting, preferably a hospital.Both RN & SW: (2) years case management experience or a similar role preferred.
