Description

Job Title: Behavioral Health Case Manager/QMHP

Non-Exempt

Reports to: Director of Behavioral Health

Salary: $55,000

SUMMARY: Case managers provide a range of services provided to assist and support individuals in developing their skills to gain access to needed medical, behavioral health, housing, employment, social, educational, and other services essential to meeting basic human services; linkages and training for patient served in the use of basic community resources; and monitoring of overall service delivery.

DUTIES AND RESPONSIBILITIES:

  • Develop an individualized service plan (ISP) based on information obtained through the comprehensive evaluation and the person receiving services' goals and preferences. The plan shall address the person's prevention, medical, and behavioral health needs.
  • Assist clients in reaching their goals outlined on their ISP during one-to-one case management sessions.
  • Follow methods for providing a continuum of crisis prevention, response, and postvention services.
  • Ensures care coordination based on a person-centered and family-centered treatment plan.
  • Assist people receiving services and families to access benefits, including Medicaid, and enroll in programs or support that may benefit them.
  • Work to coordinate care across settings and providers to ensure seamless transitions for patients across the spectrum of health services.
  • Complete all required documentation; maintain appropriate information in client files.
  • Educate community on services and program availability.
  • Conduct street outreach to target population.
  • Provide clients with grant incentives such as: food pantry and bus passes.

Requirements

EDUCATIONAL & EXPERIENCE REQUIREMENTS:

  • Bachelor's or advanced degree from an accredited college or university with a major in the human services field.
  • Must hold QMHP certification.
  • Training in trauma-informed care is preferred.
  • Experience with a variety of mental health screenings/assessments.

SKILLS & ABILITIES:

  • Strong skills in interviewing and psychological/developmental evaluation with an understanding of trauma-informed principles.
  • Ability to assess client needs through a trauma-informed lens, coordinate services, and communicate effectively in a way that prioritizes client well-being.

WORKING CONDITIONS:

  • Normal workweek schedule is Monday through Friday, 8:00-5:00 PM. Some after-hours work may be required for events
  • Work will be performed in a typical interior/office environment with a commitment to creating a trauma-informed space.
