SCHEDULE: 40, hours, Monday Friday, 8:30 a.m. 5:00 p.m., some evening hours and weekend coverage may be required.

Pays $2.00 per hour DOE

Location : 170 Morton Street, Jamaica Plain, Shattuck Campus

Program Description:

The Stabilization Program is a post-detox day treatment program that provides intensive treatment and support to 32 homeless men with a history of chronic substance abuse and homelessness.

Summary of the Position :

The Lead Case Manager provides intensive, trauma-informed, and harm-reduction focused case management to chronically homeless adults with histories of substance use and mental health concerns. The Lead Case Manager guides and supports a team of case managers in providing holistic, client-centered services, including aftercare and discharge planning, referrals, and linkages to support recovery in the community. The Lead Case Manager actively participates in staff meetings and case conferences, supports crisis prevention and intervention, and promotes best practices grounded in Motivational Interviewing and the Stages of Change theory. The incumbent will be a great team player who is also self-motivated, with solid communications skills, flexibility and creative problem solving.

Required:

  • Associates Degree in psychology or other related field such as counseling, psychosocial rehabilitation

Preferred:

  • A valid Massachusetts Drivers License in good standing
  • Bachelors Degree in psychology or other related field such as counseling, psychosocial rehabilitation
  • Advanced training in substance abuse
  • Bi-lingual Spanish/English

Knowledge/Experience:

REQUIRED:

  • Minimum of two (2) years working in case management or counseling in the human services and related fields
  • Knowledge of the issues related to substance abuse, mental illness and economic factors as precipitants to homelessness
  • Strong clinical background and familiarity with providing Trauma Informed Care and Harm Reduction Treatments to dual diagnosis populations
  • Experience with case management and implementing treatment plans.
  • Experience working with a homeless population presenting with substance use, mental health issues and a demonstrated awareness of community resources and referral agencies.
  • Experience in best practices such as: Motivational Interviewing and Trauma Informed Care
  • Strong interpersonal and client interviewing skills

PREFERRED:

  • Minimum of one (1) year in a lead case management position working with addictions, homelessness, and/or dual-diagnosed individuals.
  • Extensive experience with case management, issues of substance abuse and mental health issues, knowledge and experience using evidenced- based practices such as Motivational Interviewing and Trauma-Informed Care and access to available services for homeless men
  • Experience utilizing electronic health record documentation systems
