The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve. We are currently looking for qualified candidates for our Behavioral Health Case Manager and Behavioral Health Case Manager - Youth Focused. These two positions report to the Director of Behavioral Health and Substance Use Counseling and are regular, full Time, Non-Exempt positions with a starting hourly rate range of $29.36 to $31.46 with full benefits. Primary Objective: Under the supervision of the Director of Behavioral Health and Substance Use Counseling, the Case Manager performs screenings and short-term case management tasks for patients in the Prenatal Clinic, Adult Medicine and Pediatrics clinics. The Case Manager will endeavor to reduce the barriers patients encounter in meeting their needs around Social Determinants of Health. ESSENTIAL FUNCTIONS & RESPONSIBILITIES Direct Patient Contact: Conduct in-depth assessments, including the initial health assessment, to determine medical, behavioral, and social needs. Develop patient-centered, individualized Health Action Plans (HAPs) tailored to clients' goals, needs, and preferences. Incorporate harm reduction and trauma-informed practices into care strategies. Provide and connect to health promotion services, including health education, advocacy, and coaching. Refer and connect patients to community, housing, and social support services. Track barriers to care and assist patients in eliminating or reducing obstacles to obtaining essential services. Assist patients in completing civic and medical forms. Increase utilization of Teen Clinic and other MNHC services and improve health outcomes for teen and adult patients. Identify and coordinate collaborative opportunities with other youth and adult service providers and/or programs. Receives referrals/warm hand-offs from Behavioral Health Clinicians and cross-refers to Behavioral Health when warranted. May accompany patients to appointments on occasion. Facilitate care transitions for patients discharged from external facilities (e.g., skilled nursing, hospital, emergency room) and ensure timely follow-up with the patient care team. Administrative - Data Reporting, Program Planning & Evaluation: Maintain accurate and timely documentation of assessments, care plans, interventions, and outcomes. Develop, update and maintain list of behavioral health resources. Participate in supervision meetings and other required health center meetings. Performs other duties as assigned. Position Qualifications Requirements Education Bachelor's Degree in Social Work or related field, Or an Associates of Arts degree with 2 years of community or healthcare experience in care management activities. Languages: Fluency in English and Spanish required. Office equipment and Software: Computer and data management proficiency, including Microsoft Office applications: Outlook, Word, Excel. Proficiency with Internet Explorer and Practice Management Systems.
