Eliot is seeking a motivated Case Manager to join our Comprehensive Community Treatment Program in our Outpatient Clinic. Eliot's Outpatient Clinic provides evidence-based evaluation, counseling, and medication management, care coordination, recovery coaching and peer support for children, families and adults with mental health and substance use needs.
The ideal candidate will provide in-home support and services to clients, primarily focusing care coordination and ensuring that treatment goals are culturally competent, realistic, attainable and promote recovery.
Responsibilities:
Provide case management supports to individuals including: managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments.
Provide client support focusing on problem-solving, skills training, behavior modeling, optimism, and encouragement
Assist the team and clients in ongoing assessment of symptoms and responses to interventions
Contribute to the development of treatment plans as part of the client's treatment team
Ensure treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery
Ensure each individual is actively involved in their treatment planning process, and their preferences are incorporated
Deliver interventions in the office, virtually, in the community, and at clients' residences based on client preferences, needs, and clinical direction
Qualifications:
Bachelor's degree in Psychology, Social Work or related field
Preferred experience with multicultural backgrounds and language capacity in Spanish or Haitian Creole.
1 year experience working with population
Valid Massachusetts Driver's License
Schedule: Monday-Friday, 9am-5pm
Annual Salary
$50,000 - $50,000 USD
