What Youll Do

  • Develop individual care plans with input from the individual and their family, based on the individuals personal preferences and choices.
  • Assist individuals and families in coordinating, selecting, and overseeing their own support.
  • Work collaboratively as part of a multidisciplinary team to ensure timely integration of services.
  • Develop and implement behavioral guidelines and interventions; track and analyze related data.
  • Participate in training that promotes sensitivity to cultural and socioeconomic diversity.
  • Meet face-to-face with assigned individuals as medically necessary per their Individual Plan of Service.
  • Provide consultation to direct care staff regarding clinical and program-related matters, as applicable.
  • Provide individual and group therapy sessions, as assigned; assist in developing and facilitating group sessions.

Work Schedule

Standard Hours: Monday-Friday, 8:00am-5:00pm, on-site

Flexible scheduling available based on caseload and program needs

On-Call Rotation: Participation required; Rotates one week on, one week off

On-Call Pay Enhancements

  • Weekdays (Mon-Fri): $50 per on-call shift
  • Weekends (Sat-Sun): $100 per on-call shift
  • Holidays: $150 per on-call shift

Qualifications

Minimum Education: Bachelors degree in Social Work, Psychology, or a related Human Services field required; Masters degree in Social Work or Psychology preferred.

One or more of the following credentials strongly preferred:

  • Licensed Bachelor Social Worker (LBSW) or Limited Licensed Bachelor Social Worker (LLBSW)
  • Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW)
  • Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP)
  • Social Service Technician (SST) Certification

Preferred Experience:

  • 12 years of experience working with individuals with developmental disabilities (DD) or mental illness (MI).
  • Prior knowledge of the community mental health system and/or Medicaid-funded services.
  • Experience working with electronic medical records (EMR).
