Who We Are:

Mental Health Support Services (MHSS) is the provider of mental health, developmental disabilities, and substance use disorder services for children and adults in Chesterfield County. The Adult Substance Use Services Outpatient Program is seeking a qualified case manager within our American Society of Addiction Medicine (ASAM) Level 1.0 Outpatient Clinic. The ideal candidate for this position is an individual who has a great passion for working with individuals and their families impacted by substance use and providing support and resources to assist in their path to recovery and wellness.

Minimum Education and Experience :
Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. One year of experience providing services to individuals diagnosed with a substance use disorder is preferred; experience serving individuals with a co-occurring mental health condition is preferred; experience providing billable substance use case management in accordance with Department of Medical Assistance Services (DMAS) regulations and service definitions preferred; certification in Qualified Mental Health Professional (QMHP) preferred; Certified Substance Abuse Counselor (CSAC) preferred. Skills in navigating technology including documentation in an Electronic Health Record preferred; the ability to be flexible and respond to changing priorities preferred; knowledge of the American Society of Addiction Medicine (ASAM) Criteria preferred.

Required Knowledge, Skills, and Abilities:
Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers.

Additional Requirements :
Current valid drivers license and good driving record required. Based on the Virginia DMV point system, record must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Out of state driving records must be obtained by applicant and presented at time of interview. Records must reflect at least three years of history and be dated within thirty days of interview date.

Pre-employment drug testing, FBI criminal background check and education/degree verification required.

Duties include but are not limited to the following:
Interviews patients/clients and their family members to obtain pertinent social, economic, occupational, and physical and mental health information;
Assesses client needs, resources, determines eligibility for services and programs, and develops recommended services and courses of action;
Monitors and assesses client progress and makes recommendations or modifications to treatment plan;
Monitors delivery of services and quality of care; evaluates case plan effectiveness and manages resources;
Links individuals with other community resources;
Provides assessment, planning, linkage, monitoring, coordination, and advocacy for clients with a primary substance use diagnosis;
Completes timely documentation into an electronic health record according to licensure and DMAS standards including assessments, individual service plans, progress notes, and authorizations for services;
Collaborates within a multidisciplinary team setting to ensure excellence in customer care;
Prepares and maintains casework documentation as required in the delivery of services (e.g. case histories, assessments, treatment plans, progress notes, correspondence); and
Performs other work as required.

Monday - Friday; 8:30 a.m. - 5:00 p.m.; two required evening shifts until 9:00 p.m. on Tuesday, Wednesday, or Thursday. There is an option for either a five-day work week or a four-day work week. This position currently does not include the option for a remote work schedule. Mental Health Support Services
