Program Description REP is a community-based, intensive mental health short-term program that is centered around the priorities and interests identified by its clients--both in-custody at Santa Rita Jail and recently released into the community. REP is operated by a diverse team of clinicians and peer support staff who are committed to the values and principles of social and restorative justice in their partnership with REP's clients (TAY and adults with serious mental health conditions.) This program seeks to help increase coordination amongst service delivery systems, support clients in navigating reentry and transition of care; and improve clients' well-being to reduce recidivism and hospitalization. Job Description Under the supervision of REP Director and REP Clinical Supervisor, REP clinicians provide intensive short-term mental health and case management within the context of a multi-disciplinary team. REP clinicians are empathic, flexible, growth-minded, and their praxis clearly demonstrates understanding of external factors that impact clients' recovery and their [clinician's] own positionality, trauma-informed care, and use of harm reduction. This role embodies the practice of meeting people where they are at, including meeting individuals at various community settings, in Santa Rita Jail, honoring where people are at in their recovery journey, flexibly/creatively responding to needs, etc. Responsibilities also include participating in the rotation of a 24/7 on-call line and clinical documentation. Specific Responsibilities Provide intensive mental health (individual/group therapy, rehabilitation, diagnostic assessments, crisis) and case management for REP clients for a period of 6 months to address mental health and other co-occurring conditions and to support clients with achieving their self-identified goals. As needed, provide outreach/in-reach services to support client engagement and/or successful reentry into the community. Elicits and maintains strong relationships with clients and their care team which may include other professionals and client identified support persons. Demonstrates humble leadership as clinical lead on clients' teams, including providing consultation to REP team. Accurately and timely documents all services provided in accordance with Alameda County, Medi-cal, and Felton standards. Completes other administrative tasks, such as progress reports and treatment plans for specialty courts. Provides de-escalation and crisis prevention, intervention, and management, including participating in coverage of REP's 24/7 on-call line (stipend provided.) Minimum Requirements and Qualifications Masters degree or higher in Social Work, Counseling, Psychology, or related clinical field with active BBS-registration in California. Minimum 2 years of experience providing mental health and/or case management for TAY and/or adults impacted by homelessness, carceral and/or foster care system, substance use disorders, and/or moderate-severe mental illness. Demonstrated outreach, rapport building, and engagement experience, particularly with those who are struggling to be active in their own recovery. Willingness to work from a strengths-based, harm reduction, client-centered model and in close collaboration with members of REP team. Strong skills in working independently and managing multiple tasks and deadlines. Ability to follow instructions and work under the direction of others. Ability to communicates clearly and compassionately with colleagues, clients, and supervisory staff verbally and in writing. Working knowledge of social service resources in Alameda County for TAY and adults and of the DSM 5 (highly desired). Strong writing skills and accountability for work.
