Position Summary:

The Housing Navigation Center Case Manager II provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual client needs. Responsible for conducting assessments for homeless clients and clients on the verge of homelessness to create a client centered individual service plan which identifies supportive services needed to transition to an independent life. Additionally, responsible for the necessary follow up with local agencies to assure needed services are initiated and continue as planned.

Essential Responsibilities:

  • Conduct intake with clients who meet strict program criterion and enroll via HMIS/Bridges.
  • Conduct assessments to identify the assistance needed and develop individualized service plans for each client.
  • Designing structures to assist clients in enhancing their ability to focus, make effective changes, achieve desired goals, and maximize success and fulfillment in life.
  • Maintain awareness of potential resources in the community and make proper referrals.
  • Follow through with referrals made to various agencies to ensure the reasonable outcome of the referrals.
  • Identify client's eligibility to various housing, employment, substance use, nutritional assistance or other applicable programs available in the community and provide appropriate program guidelines to clients.
  • Meet with clients regularly to ensure goals are being met.
  • Accurately document case notes consistently and in great detail.
  • Protect all confidential information, company property and electronic data. Comply with safety rules.
  • Other related duties as directed.

Knowledge, Skills and Abilities:

  • Must be computer literate and possess effective written and verbal communication skills.
  • Ability to work independently or with a group.
  • Must have a strong sense of focus and must be task-oriented and non-judgmental with a clear sense of personal boundaries.
  • Must have a respect for confidentiality involving both clients and employees.
  • Ability to work in a variety of settings with culturally diverse groups. Ability to be culturally sensitive and appropriate.
  • Must be able to manage personal appointments and schedule, prioritize work and meet necessary deadlines.

Qualifications:

  • Bachelor's degree in social service field from an accredited college or university preferred, or equivalent combination of education and experience.
  • Two years of full time professional level casework experience in a social service agency preferred.
  • Prior experience with homeless population required. Experience with chronically homeless men preferred.
  • Knowledge and adherence to Social Work or Counselor Code of Ethics.
  • Flexible schedule to work after hours and weekends. Regular schedule may include either Saturday or Sunday each week.

Physical Requirements:

  • Primarily works in office environment, but will also work throughout the agency and off site.
  • Ability to regularly communicate with clients, co-workers and supervisors.
  • Ability to operate and use office equipment, especially computer, keyboard and mouse.
  • Ability to continuously sit and/or stand. Ability to walk continuously if needed and climb stairs as needed.
  • May occasionally lift up to 25 pounds.

This position pays $20.00 per hour.
