The Medical Case Manager provides outreach, care coordination, and support services to individuals living with or at risk for HIV/AIDS, ensuring access to comprehensive healthcare and community resources.
Direct Care & Case Management• Conduct outreach in collaboration with community partners to identify individuals in need of services
• Connect persons served to medical care, insurance, and social, mental health, and financial supports
• Perform ongoing assessments and develop individualized care plans
• Provide referrals and resource navigation based on client needs
• Conduct home visits and transport clients to appointments (medical, legal, social services, etc.)
• Provide health education, interpretation, and assistance accessing services
• Maintain accurate documentation and data entry in CareWare or other EHR systems
• Participate in community meetings focused on health access and education
• Collaborate with HIV Services team on prevention strategies (HIV/HCV/STI, overdose prevention)
• Screen, assess, and counsel individuals at risk for HIV/HCV/STI or opioid overdose
• Collect specimens for testing (venous blood draw, finger stick, oral swab, urine specimen)
• Stay current on HIV, STI, Hepatitis, substance use, and harm reduction best practices through trainings
• Assist with operations of the BAMSI Community Food Pantry (inventory, ordering, pickup, distribution)
• Build and maintain professional relationships with clients, staff, and community partners
• Follow all agency policies, procedures, and safety protocols, including universal precautions
• Maintain flexibility to work occasional evenings and weekends as needed
• Perform additional duties as assigned
• Associate or Bachelor’s degree in a human services field preferred , OR
• Minimum of 2 years professional or volunteer experience in a related field required
• Case management or care coordination experience required
• Experience working with individuals living with HIV/AIDS
• Experience working with substance use/addiction populations
• Familiarity with harm reduction principles
• Strong communication, organizational, and multitasking skills
• Ability to work independently and collaboratively in a team environment
• Cultural competency and comfort working with diverse populations
• Bilingual abilities (Haitian Creole, Cape Verdean Creole, Portuguese, Spanish) strongly preferred
• Excellent documentation and computer skills (Microsoft Word, Teams, Outlook)
• Phlebotomy skills preferred or willingness to learn
• Valid driver’s license and reliable transportation required
• Residency in or familiarity with the Brockton area preferred
