Case Manager

Responsibilities Include:

Conduct assessments to determine client needs and develop individualized service plans.
Coordinate with community resources to provide clients with access to necessary services.
Maintain accurate and up-to-date client records and documentation.
Advocate for clients and assist them in navigating social services systems.
Facilitate group sessions and workshops to support client development and well-being.
Collaborate with shelter staff and external agencies to ensure comprehensive care.
Bachelor's degree in social work, psychology, or a related field preferred.
Experience working in a homeless shelter or similar environment.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Knowledge of community resources and social services systems.
