Travel Medical Social Worker - $2,069 per week in Everett, WA

Start date is ASAP. The position requires working four 10-hour shifts per week. Candidates must have strong acute care experience and discharge planning skills. A diverse case management experience is preferred, including Med Surg and LLOS. First-time travelers are accepted. Weekend work is required, every other weekend. Candidates must be willing to float between like units as needed. Epic charting system experience is required. Candidates must be comfortable working in all areas of the facility. The average case load is 18-24 patients, potentially up to 30 on weekends. Applicants must have experience with various patient demographics, including adults and geriatrics. Interdisciplinary team support includes physical therapy, pharmacy, and social services. Knowledge of cultural issues and patient advocacy is important. The ability to coordinate services for home care and equipment following discharge is required. Candidates should possess skills in conducting initial evaluations and needs assessments. Documentation of care delivery and effective communication with team members are essential. Must have knowledge of advance directives and guardianship issues.
