GA Family Services - Regional Permanency Center (RPC) Caseworker Reports To: Community Services Program Manager
Position Status : Full-Time (40-hours per week)
Position Schedule: T he standard schedule is Monday through Friday, hours will vary based upon client and program needs
Salary Range: $21- $26.66/hour, based on education and experience
Make a Difference in the Lives of Children and Families GA Family Services is seeking a compassionate, dedicated, and motivated Post-Adoption Program Case Worker to support children, adoptive families, and guardianship families throughout Chautauqua, Cattaraugus, Erie, Wyoming, and Niagara Counties .
This grant-funded position through the New York State Office of Children and Family Services (OCFS) plays a vital role in helping families build stability, strengthen relationships, and access critical resources that promote long-term success and well-being.

Position Overview The Post-Adoption Program Case Worker provides comprehensive case management and support services to children and families involved in post-adoption and post-guardianship services. This role focuses on strengthening family functioning, promoting permanency and stability, and enhancing each child s ability to thrive at home, in school, and within the community.
The Case Worker manages an assigned caseload and works closely with families, schools, service providers, and community partners to ensure individualized and effective support.
Qualifications Bachelor s Degree in Human Services or a related field required; Master s Degree preferred
Minimum of two years of direct service experience working with children and families preferred
Valid driver s license, reliable transportation, and vehicle insurance as required by New York State law
Strong communication, organizational, and interpersonal skills
Experience working with children and families preferred
Knowledge of trauma-informed and strength-based practices preferred
Bilingual in English and Spanish preferred, but not required
Key Responsibilities Conduct screenings, assessments, and ongoing case management for post-adoption and post-guardianship families
Develop and maintain individualized service plans focused on family stability and child well-being
Provide support, education, advocacy, and crisis intervention to children, caregivers, and families
Collaborate with schools, community providers, and interdisciplinary team members to coordinate services
Connect families with counseling, behavioral health, and community resources
Maintain accurate and timely documentation, reports, and case records in compliance with agency and OCFS requirements
Participate in team meetings, supervision, trainings, and on-call responsibilities
Support agency initiatives, outreach efforts, and evolving program needs
G.A. Family Services (GAFS) Organizational Responsibilities:
Maintain professional, respectful, and collaborative relationships with clients, families, and staff
Demonstrate ethical and professional conduct while upholding confidentiality standards
Communicate effectively both verbally and in writing
Participate in regular supervision and performance evaluations
Support agency mission, values, and organizational goals
Uphold child and family rights in accordance with OCFS mandates and agency procedures
Follow all agency safety policies and procedures
