Key Responsibilities:
Support families in accordance with New Hampshire state regulations and waivers, fostering independence and community engagement
Advocate for individuals by responding to changing needs and utilizing formal and informal resources effectively
Serve as a liaison between families, community agencies, and service providers to ensure seamless service delivery
Coordinate assessments, evaluations, and transitions (hospital discharges, moves, etc.) ensuring timely and comprehensive planning
Facilitate legal processes such as Guardianship, Power of Attorney, and Advance Directives
Manage budgets responsibly using NH EASY for funding requests and service planning
Prepare and maintain all required documentation, including Service Agreements, Action Plans, and contact notes
Monitor service quality, ensure compliance, and support person-centered planning principles
Maintain ongoing communication with families, individuals, and professionals to ensure their needs are met
Provide coverage and support to team members during absences as needed
Stay informed and engaged through staff development, council meetings, and ongoing education efforts
Required Skills & Qualifications:
Bachelor's degree in a related field (preferred), or equivalent extensive human services experience
Minimum of 2-3 years' case management experience, with preference for experience in developmental disabilities
Strong verbal and written communication skills, with a friendly and professional demeanor
Proficiency in MS Office (Word, Excel, PowerPoint) and willingness to learn new programs
Excellent organizational skills with the ability to multitask and meet deadlines
Valid driver's license, reliable transportation, and proof of auto insurance
Ability to work in various environments, including facilitating transportation and emergency assistance
Successful completion of criminal record, MVR checks, and TB testing
Working Conditions:
Office environment with regular travel throughout New Hampshire
Ability to sit, stand, kneel, and bend as needed
Ability to lift up to 20 pounds and perform physical activities related to supporting individuals and families
Exposure to diverse living situations, including pets and smoking environments
