Develop and conduct an intake with each new client that includes an assessment of needed supports and services, available resources, and a plan for implementation.
Provide referrals and linkages with other agencies as appropriate.
Support clients in implementing plans of action.
Provide advocacy for clients applying for services, including entitlement benefits, housing, legal proceedings, and other types of assistance.
Make clients aware of Crime Victim Compensation through the Office of Victim Services and to assist clients in applying for benefits
Case Manager Minimum Salary: $ 20.00/hour
Case Manager Maximum Salary: $ 20.00/hour
Case Manager Schedule: Monday - Friday: 8:30AM to 4:00PM
Qualifications:
Associates Degree in Human Services or related field
1 - 3 years of experience working with a diverse population
Valid driver s license, proof of current car insurance and registration and access to a reliable, working vehicle
