Position Description:

The Comprehensive Assisted Recovery & Empowerment (CARE) Program Case Manager willplay a critical role in managing patient care, improving outcomes, and coordinating with various members of the healthcare team. The Case Manager will work with the healthcare team and community partners to develop and implement treatment plans that incorporate both medical and behavioral health interventions to ensure that patients receive the appropriate level of care based on their individual needs.

The CARE Program utilizes a team-based collaborative care model to support patients with co-morbid mental health disorders and substance use disorders or chronic pain in the primary care setting. The goal of the CARE Program is to improve both mental and physical health. Through intensive case management, the Case Manager contributes to improving patient outcomes and enhancing the overall quality of care provided by the Forest Home Health Center Family Medicine clinic.

The ideal candidate for this position will be a highly motivated and compassionate healthcare professional with a passion for improving patient care outcomes. Support caseload of patients enrolled in Comprehensive Assisted Recovery & Empowerment Program for collaborative care case management, working closely with the healthcare team to ensure seamless communication and coordination between team members.

Responsibilities:

Maintain tracking dashboard for high-risk patients, identifying critical follow-up visits.

Collaborate daily with physicians and clinical staff to maximize quality care.

Communicate plan of care to patients including responding to telephone messages.

Utilize electronic medical records (EMR) and tracking systems to manage patient information and workflow efficiently; proficiency in EPIC is preferred.

Support behavioral management and behavioral change skills through active and consistent phone calls with patients

Monitor patient care to ensure patients progress towards desired outcomes.

Coordinate patient follow-up and ongoing clinical management.

Troubleshoot patient care barriers, working closely with the social work, RN triage, behavioral health, and physician team members.

Facilitate Collaborative Care meetings with CARE team and psychiatry.

Assist with the fiscal management of annual budgets.

Provide outreach/education services to other professionals as appropriate.

Collaborate with community partners.

Knowledge - Skills - Abilities

Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting.

Medical documentation, interpersonal, practice-based learning and improvement, empathy, and decision making skills.

Preferred Schedule:

8:00AM -4:30PM - Hybrid

Position Requirements:

Minimum Qualifications:

Appropriate experience may be substituted for education on an equivalent basis.

Minimum education: Bachelor's degree in nursing or social work

Minimum experience: 3 years

Certifications: Active RN or Social Work license

Preferred Qualifications:

Preferred education: Master's degree in nursing or social work

Preferred Experience: 3 years of experience working with patients who have substance use disorders and mental illness. Strong communication and interpersonal skills. Working knowledge of Epic Compass Rose is a plus; willingness to learn is acceptabletraining will be provided.
