The Franklin County Community Based Correctional Facility (CBCF) is a minimum security, American Correctional Association (ACA) accredited, residential prison diversion program for adult male offenders. The Franklin County CBCF is a 180-bed facility that houses adult male offenders (referred to as "residents") for a maximum of 180 days. The CBCF believes community safety is of upmost importance. Second is targeting residents' criminogenic needs via intense cognitive behavioral programming in an effort to reduce their likelihood of recidivating. Position Summary and Purpose:

The Case Manager at the Franklin County Community-Based Correctional Facility (CBCF) is a direct care, essential position responsible for assisting residents in successfully navigating and completing the program. Reporting directly to the Reentry Manager, this role is responsible for identifying individualized resident needs, collaborating with residents to assist them in developing Individualized Success Plans, and tracking resident progress throughout their program. Additionally, Case Managers are responsible for providing interventions and updating residents Success Plans to navigate changes and barriers to completion as they occur. The Case Manager will support the facility's mission by fostering a collaborative environment focused on rehabilitation and successful reintegration while also ensuring that practices are in accordance with established rules, policies, and procedures.

General Responsibilities / Essential Job Functions:

  • Conducts office visits with residents to identify needs and track progress.
  • Administers assessments to identify criminogenic needs including, not limited to, ORAS, PREA, TCU Drug Screen, Criminal Thinking Scale, and ODARA assessments.
  • Collaborates with residents to develop and execute individualized Success Plans based on assessed levels of risk, need, and responsivity (RNR).
  • Tracks resident dosage hours to ensure compliance with agency and regulatory standards.
  • Regularly updates Success Plans with residents to track and measures progress toward established goals and objectives.
  • Facilitates resident phase ups and completions by administering phase up tests and providing treatment input.
  • Attends and participates in weekly Treatment Team meetings to staff resident behaviors and progress.
  • Collects and reviews resident Structured Time Sheets.
  • Maintains adequate documentation in resident case record to ensure compliance with agency and regulatory standards.
  • Administers and collects resident Exit Surveys prior to completion
  • Completes and reviews resident mail, phone, visit lists.
  • Collaborates with courts and other legal referral sources to ensure compliance with legal mandates.
  • Reviews and audits resident case records prior to final submission.
  • Issues effective approval and disapproval to residents and assists in ensuring residents meet 4:1 ratio.
  • Assists residents in developing and implementing problem-solving skills and emotion regulation strategies to navigate programmatic barriers.
  • Assists in planning and overseeing specialized visits, resident family activities, and events.

Additional Duties:

  • Maintains knowledge of effective intervention techniques and evidence-based treatment approaches.
  • Assists in resident transportation as needed.
  • Maintains compliance with agency policies and regulatory standards including, but not limited to, American Correctional Association (ACA), Bureau of Community Sanctions (BCS), and Prison Rape Elimination Act (PREA).
  • Attends and participates in mandatory departmental and/or agency meetings.
  • Performs other duties as assigned.

Other Job Requirements:

  • Completes a minimum of forty (40) hours of in-service and twenty-four (24) hours of COB training per year. Participates in seminars and continuing education programs when appropriate and feasible.
  • Completes educational/training hours that may be required to maintain certification.
  • Assists in facility coverage as needed.
  • Hold and Maintain CPR/First Aid Certification
  • Assists in facility searches, resident searches, and resident urinalysis as needed.
  • Serves as a representative of the CBCF when directed.
  • Valid driver's license required.

Minimum Qualifications and Skills:

  • Associates Degree (required) or Bachelor's Degree (preferred) in social work, sociology, psychology, criminal justice, or a related field
  • Two (2) to Three (3) years of experience of corrections, psychology, criminal justice, social work, substance use disorder treatment, or mental health field (preferred)
  • Strong interpersonal and conflict-resolution skills
  • Strong oral/written communication skills
  • Knowledge of community-based corrections and therapeutic alliance preferred.
  • Ability to analyze data and recommend solutions to improve outcomes.
