Job Description


The Staff Pad is proud to partner with St. Peter's Health in Helena, Montana, in the search for a dedicated RN Care Manager. This vital position offers the opportunity to influence patient care outcomes and support smooth transitions across the continuum of care within a mission-driven organization.

Position Overview:

As a CM/UR RN, you will collaborate with patients, families, physicians, and interdisciplinary teams to guide patients through their Emergency Room and acute care journeys. You'll manage utilization reviews, ensure the appropriate level of care, and create safe and timely discharge plans. This role blends clinical expertise, advocacy, and communication to ensure patients receive the right care at the right time in the most cost-effective way.

Key Responsibilities:

Access Management/ED Coordination:

  • Partner with medical staff to determine correct admission status and treatment plans

  • Meet with patients/families to assess navigation and transition needs

  • Coordinate reviews and liaise with E.H.R. physician consultants

  • Identify and help resolve financial or transition barriers

  • Use high-risk screens to flag complex cases and coordinate with social work

Progression of Care:

  • Support adherence to evidence-based clinical protocols

  • Coach physicians in documentation best practices and compliance

  • Join interdisciplinary rounds to advocate for effective treatment plans

  • Initiate referrals and manage discharge services such as home health, DME, or hospice

Transition/Discharge Planning:

  • Identify patients with complex psychosocial needs and coordinate social services

  • Guide safe discharges or transfers to alternate levels of care, including Swing Bed status

  • Communicate plans and transitions clearly to the receiving CM team when applicable

Rotational Responsibilities Include:

  • Utilization Review: Conduct daily insurance certifications and patient reviews

  • ED Case Management: Assess admission necessity and support outpatient referrals

  • Discharge Planning: Assist inpatient units with discharge logistics and coordination

Qualifications:

  • Experience: Minimum 3-5 years of acute care experience required

  • Education: Registered Nurse (RN) required; BSN or equivalent preferred

  • Licensure: Current Montana RN license

  • Preferred Certifications: Case Management and/or Utilization Review certification

Skills & Attributes:

  • Exceptional communication, negotiation, and time management skills

  • Proficiency in Word, Excel, and familiarity with Meditech preferred

  • Ability to work independently and collaboratively across departments
