JOB SUMMARY:
Under general supervision or direction, address various challenges within the aging process, promoting independence, autonomy and dignity in later life. Provide therapeutic services to Laguna Woods Village residents that include: individual short-term therapy, clinical case management, clinical assessment screenings, biopsychosocial assessments and support groups under the clinical supervision of an LCSW. Performs related work as required. Work involves direct clinical services to Laguna Woods Village residents including: individual short-term therapy, clinical case management, clinical assessment screenings, biopsychosocial assessments and support group facilitation under the clinical supervision of the Social Services Manager.
ESSENTIAL FUNCTIONS : The social worker will conduct initial in-home comprehensive assessments. Maintain and develop a thorough knowledge of community resources, programs and services that serve older adults. Social worker will develop and implement client individualized plan of care to address unmet needs. Will assist residents in accessing appropriate community resources. Social worker will provide case management, education, and assistance in the overall coordination of services to Village residents. Social worker will deliver short-term individual Cognitive Behavioral Therapy (CBT) therapy sessions. Position requires development and facilitation of therapeutic support group. Social worker will provide leadership and supervision to assigned MSW interns when applicable. Compile and report case load statistics in a concise, timely manner to produce a monthly report is necessary. Other duties will be assigned by supervisor as necessary. Social worker will drive personal vehicle to residents’ homes, meetings, workshops and other job-related responsibilities. Regular attendance required. All duties performed on-site for the Community. EQUIPMENT OPERATED : Personal vehicle, computer, printer, copier, fax machine and telephone.
WORK ENVIRONMENT : While performing the duties of this job, the employee is in a climate-controlled indoor office, residents’ homes and normal outdoor elements.
DESIRABLE QUALIFICATIONS : Graduation from an accredited college or university with a Master’s degree in social work (MSW) and a current clinical social work license (LCSW) issued from the state of California or become licensed within three years of employment. • Knowledge of the occupational hazards and safety precautions of the job. • Experience in delivering therapy services preferred. • Ability to develop a sense of trust and rapport with program’s clients. • Ability to work effectively with diverse populations and in team settings. • Outstanding communication skills. • Excellent analytical, problem solving, and organizational skills. • Ability to work independently to achieve desire goals managing time efficiently. • Ability to maintain objectivity and confidentiality in documenting and discussing caseload. • Ability to establish and maintain effective working relationships with others. • Knowledge of modern office equipment, practices and procedures with basic computer skills. • Knowledge of Orange County community services and resources desired. • Valid California driver’s license, reliable transportation. • Submit and successfully complete a LiveScan fingerprinting background check.
SPECIAL REQUIREMENTS : Possession of a valid and current California driver's license. A current clinical social work license (LCSW), issued from the state of California or become licensed within three years of employment.
PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is frequently required to walk, talk, hear, see, and use hands to finger, grasp, feel and reach. The employee is regularly required to sit for long periods of time (up to 6 hours) and use repetitive motion to operate a keyboard for long periods. The employee is occasionally required to kneel, pull, push, stoop, and lift and carry up to 20 lbs.
