Job Description


Direct Care Duties:

This position ensures appropriate and meaningful habilitation services for developmentally disabled individuals receiving 24-hour support from Live-in Direct Support Professionals in a Group Home setting. The scope and sequence of the habilitation program is developed and incorporated into the plan of care by an interdisciplinary team. The team may include the client, family, Team Care Manager, Direct Support Professionals and others who may help identify the client's needs and design appropriate services.

Qualifications

The qualified applicant must possess the ability to work effectively for and with individuals with developmental disabilities and have the following skills:
Baccalaureate or higher degree in a related field or at least two years of documented experience, relevant to the population of residents to be served, as a care provider, with documented skills or training relevant to the population of residents to be served.
Be able to work effectively for and with individuals who have intellectual and behavioral differences.
Effectively support service recipients to develop a community network in their community.
Be able to act independently and function as part of an overall team.
Keep accurate service delivery records.
Good judgment and reasoning skills a must. Good communication skills and a willingness to listen.
Approved Criminal History Check by the Alaska Background Check Unit.
Current TB test, CPR/First Aid Certification.
At least 21years of age.
Ability to speak, read and write in English.
Proven organizational skills, prioritizing tasks and time management skills.
Internet skills and basic use of Word and other applications. Will train.
Use smartphones/tablets and installed apps. Will train.
Ability to complete basic arithmetic computations and perform tasks such as balancing a checkbook and recording expenditures.
Ability to follow oral and written instructions.
Knowledge of community resources.
Ability to work flexible hours, including evenings, weekends, overnights, and holidays.
Have an valid driver's license and clean driving record.

Specific duties include:
Work a minimum of two live in shifts per week.
Assist people who receive our services to share places and develop a positive reputation and status with people in their community.
Assist service recipients in making healthy and safe choices that affect their life in their community, while respecting their right to choose how they live.
Assist service recipients to grow in their relationships with others in their community.
Provide direct training and related support services to people who receive our services.
Understand and adhere to CMS final settings rules.
Activities of Daily living:
Dressing including appropriate selection of weather related clothing, dressing where needed and shoe tying.
Assistance with hair care, nails care, oral care, and toileting when needed.
Assistance in showering and shampooing, if needed.
Meal planning, preparation and eating regulation.
Preparing for day program, bedtime routine, and waking. Household maintenance:
Household duties including laundry, sweeping, vacuuming, dusting, dishes, bedroom, bathroom and kitchen at least once each week, or as needed. Heavy cleaning completed monthly.
Grocery shopping as scheduled.

Health Maintenance:
Medication: Assist with the self-administration of medications and treatments per doctor's orders and chart accurately.
Provide basic first aid as needed.
Communicate health issues to the Coordinator and Conservator/Family.
Schedule and attend medical appointments with the person supported as appropriate and directed.
Read, implement and adhere to a behavioral plan. Safety/Security
Security: Check emergency equipment weekly. Check locks daily.
Safety: Check for potential safety hazards daily.
Review safety rules for each service recipient to ensure their specific needs are met.
Assist with contacting landlord or the Coordinator for needed emergency repairs.

Administrative duties:
Accurately and completely fill out Documentation.
Daily documentation - ISPs, checklists, medication records, T-Logs, and time sheets
Other documentation as required (appointments, event reports, blood pressure/seizure/glucose levels)
Attend training as scheduled.
Participate in team meetings and supervision.

Supervision Duties
Mentor Direct Service Professionals in the implementation of training programs for developmentally disabled people receiving habilitation services through the Division of Senior and Disability Services.
Mentor, model, and ensure adherence to CMS final settings rules.
Review daily ISP's for accuracy and meeting minimum standards, approve ISP's as ready or contact DSP's for correction. Ensure all notes are entered, correct, and audited within the timelines required by the State of Alaska and Crossroads.
Review additional documentation to ensure completion and accuracy (medication records, appointments, recording of blood pressure, glucose levels or seizures).
Ensure implementation of care plan by training DSP's in the use of current and correct training techniques.
Ongoing contact and coordination with families'/guardians regarding services as needed.
Directly supervise DSP's and monitor performance.
Perform direct care duties as scheduled and needed.
Observe and direct safety precautions and security procedures.
Keep a variety of records and compile reports concerning resident training, care and treatment as needed.
Coordinate with Training Director to ensure required DSP trainings are current.
Review GER's for accuracy and correction.
Primary point of contact with Team Care Manager.
To interview and hire DSP's that have characteristics outlined in the Code of Ethics and have the potential to become long term employees.
Complete staff scheduling.
Ensure schedules are current in Therap at all times.
Assist with scheduling leaves of absence.
Schedule themselves to work at least 2, but no more than 3 live-in shifts a week.
Assist with finding coverage for call-outs, as needed.
Maintain all Assisted Living Home (ALH) records at all times.
Ensure cleaning standards according to 7 AAC 10.1040 are maintained at all times.
Ensure the home maintains Universal Precautions as outlined in 7 AAC 10.1045.
Ensure Caregiver Hygiene standards are met according to 7 AAC 10.1050 at all times.
Complete the required licensing documentation and maintain the ALH license.
Conduct monthly fire drills, quarterly evacuation drills, maintain residence files and conduct quarterly ALH review of each resident.
Fill in on an emergency basis, as needed.
Other duties as agreed upon by the Team Care Manager and the Executive Director.
