The Case Manager Opportunity:

The Case Manager provides community-based case management for individuals diagnosed with mental health and or substance use disorders. The Case Manager coordinates care individuals enrolled in Comprehensive Community Services, Community Support Program, Children’s Long-Term Support, or the Coordinated Services Team program by collaborating with a multi-disciplinary team. The population served includes individuals with a mental health and/or substance abuse diagnosis.

Compensation and Benefits:

  • The starting pay range for this role is $26.50-$30.85, based on education and experience.
  • This job is full-time and in person, working 8:00am-4:30pm on weekdays.

  • As a full-time employee of NCHC, you are eligible for the Wisconsin Retirement System (WRS). The WRS is a hybrid defined benefit plan. It contains elements of both a 401(k) or defined contribution plan and a defined benefit plan. Participating staff contribute 6.95% and NCHC will match this at 100% each pay period!

What Qualifies You:

    • Bachelor’s or Master’s degree in human services field
    • At least one year of case management experience.
    • Ability to work in fast-paced environment.
    • Person-centered service in all interactions.
    • Valid driver’s license and use of a personal vehicle.
    • Attention to detail and organized.

What a Case Manager does at NCHC:

  • Determine ongoing eligibility for Community Treatment programs via the approved state functional screen
  • Facilitate the assessment and service planning process including the identification of consumer needs, goals, objectives, strengths, barriers, interventions, and their priority in the treatment planning process.
  • Assess ongoing needs, including crisis situations, to determine treatment interventions and service provision.
  • Ensure the service plan and service delivery for each client is integrated, coordinated, and monitored
  • Provide services in adherence to the pillars of Community Treatment program philosophy which include being strength-based, recovery-oriented, person-centered, flexible, trauma informed and consistent with best practice.
  • Complete documentation in compliance with Community Treatment standards, agency policy and applicable regulations.
  • Adhere to scheduled work hours with regular and predictable attendance.
