Summary
Job Description:
Case management services are designed to assist patient and family members with continuing care arrangements that may be necessary following discharge from the hospital. Case managers work with physicians, nurses, physical therapists and other members of the health care team to identify and coordinate alternative care or services that will assist in recovery.

Job Description:
- The Case Manager strives for excellence in direct patient care by building a collaborative caring work environment coordinating care and information across the various disciplines focused on patient outcomes and wellness.
- The Case Manager assesses plans and facilitates options and services which best meet the patients healthcare needs in the continuum of care.
- The Case Manager formulates goals with the patient and their families.
- The Case Manager advocates for the patient through communication and evaluation of available resources to promote quality cost-effective and safe outcomes.
- The Case Manager incorporates standards of care for Case Management including identification/selection of patients for case management services, problem identification, planning, monitoring, evaluating, transition of care and outcomes.
- The Case Manager implements practice based on current research and evidence based practice and ensures adherence to hospital and department policies, regulatory standards, and guidelines.
- The Case Manager supports an environment of caring focused on the patients healing and wellness.
Job Requirements:
- Bachelor's degree in Nursing
- Current RN license in Ky
- Five years experience in acute care nursing, case management, utilization management or related field.
- BLS
