Pay is commensurate with experience within a range of $20.00 to $27.00 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primaryresponsibility of the Case Manager-Strengths is to coordinate case management servicesfor adults in the community with recurrent mental health issues, developindividualized treatment goals, and aid in accessing treatment. Specific dutiesinclude but not limited to:

  • To helpclients to understand the symptoms of their mental illness and to minimize theeffect of those symptoms in all the significant areas of their lives.
  • Assist clients in learning and practicing newskills that contribute to their recovery as specified in the treatment plan.
  • Providesupportive counseling, solutions focused interventions; behavioral analysis toincrease client's ability to function in all life domains.
  • Providing outreach and crisis interventionservices to clients and prospective clients to enable timely access to thoseservices appropriate and necessary for crisis stabilization with the purpose ofdiverting the client from hospitalization and allowing the client to maintainindependent living;
  • Complete strength assessment with client andidentify strategies or treatment options associated with the consumer's mentalillness with the goal of minimizing the negative effects of mental illnesssymptoms or associated environmental stressors.
  • Develop individualized crisis plan withclient and review during treatment plan review time.
  • Additionalresponsibilities that may be assigned as circumstances warrant specificallyrelated to supportive housing facilities and clients; and
  • Regular and predictable attendance is anessential function of the job.

Qualifications: The Case Manager-Strengths positionrequires but not limited to:

  • BA/BS degree or four years of equivalenteducation and/or experience working in the human services field.
  • Reliabletransportation; and a valid, unrestricted driver's license and proof of currentcar insurance
  • Strong organizational skills
  • Strong oral and written communication skills
  • Strong interpersonal skills in working withdiverse publics
  • Flexibility in assigned working hours

Bert Nash Center requires proof of Covid-19 immunization prior to starting any position. Covid-19 vaccine series must be completed or in process to be cleared for new hire orientation.
