Essential duties include: Conduct assessments on patients Develop and implement the hospital's plan of care Educate patient and family on the plan of care Maintain accurate and updated medical records on patients Qualifications: Current Registered Nurse (RN), Licensed Practical Nurse (LPN) or Social Worker (MSW) in the State of Louisiana Minimum of 2 years experience in an inpatient rehab or skilled nursing facility Compassionate team member with time management skills Exceptional People providing Exceptional Care.
