TITLE: Case Manager, OCC Community Justice Support Center JOB CLASSIFICATION: Non-Exempt
REPORTS TO : Director, OCC CJSC
JOB SUMMARY: The Case Manager is responsible for the overall implementation, assessment, and administration of quality casework services at the Center. S/he will display and encourage the character traits of honesty, respect, caring and responsibility in the fulfillment of his/her job duties.
JOB FUNCTIONS:Program/Department Responsibility:
- Ensure that all casework services are consistent with the operating policies of the Office of Community Corrections (OCC) and other regulatory agencies.
- Ensure that participant documents such as a copy of all intake materials are placed in participant files including those provided by supervising agencies (e.g., Probation, Parole, and Sheriffs Department).
- Complete intake assessments utilizing assessment tools (e.g. LS/RNR, TCUDS, etc.); assess risks/responsivity needs; and develop treatment plans for each assigned participant.
- Ensure that treatment plans and goals for transition are completed and reviewed on a monthly basis. Provide feedback to participants.
- Make necessary referrals to outside agencies or departments as needed. Case manage community based-services.
- Complete regular check-ins with participants.
- Document participant interaction (i.e., client notes) as needed.
- Assign and track completion of Client Behavioral Interventions (CBIs) as needed.
- Create an after care/discharge plan for each assigned participant.
- Facilitate CBT manualized groups to fidelity. Submit to/conduct group audits.
- Utilize technology for programming needs (i.e., virtual meetings/groups or hybrid models).
- Keep participant data and information updated in the system (Client Information Management System or CIMS).
- Transport or conduct drug testing on participants upon staffing deficiencies.
- Participate in all meetings and trainings as requested by the supervisor(s).
- Perform other duties as assigned or directed by the supervisor(s).
- Collection and documentation of urine screens as needed.
- Attend all assigned meetings.
- Actively participate in the Ys Annual Campaign.

Education/Experience/Physical Requirements:- Bachelors Degree from an accredited college or university.
- Two (2) years of supervised counseling experience or four (4) years of experience delivering manualized cognitive behavioral treatment.
- Must be willing to work with high risk populations including substance abusers, court involved individuals, and trauma survivors.
- Preferred licensed by professional board of practice in counseling, mental health, social work, or alcohol and drug abuse.
- Good oral, written, and presentation skills.
- Strong computer skills including Microsoft Word, Excel, Teams, Outlook and Power Point.
- A minimum of one evening shift per week is required.
- Some traveling required for meetings and trainings.
- Must have a valid drivers license in good standing.
- Must submit and pass drug testing upon hire.
- Must be able to sit, stand, bend and walk during the work day.
