The Case Manager provides case management services to assigned clients, providing information and resources for clients to help them manage their basic daily needs, coordinate appointments between organizations, provide education about community resources to clients and families.

Provides case management and supports Coordination and Community Living Supports (CLS) /Respite services for children and families on an assigned caseload in accordance with all applicable contractual and regulatory requirements and accreditation standards.
Duties and Responsibilities
Job Qualifications
Education, Training, and Licensure/Certification
- Bachelor degree in human service related field, Masters Degree preferred
Experience
- One year of experience working with individuals having disabilities, mental illness, and/or children in a residential setting
Knowledge Skills and Abilities
- Ability to effectively manage crisis situations and solve problems; ability to use de-escalating techniques. Excellent organization and prioritization skills; ability to utilize computer technology and various applications, including spreadsheets
- Willingness and ability to demonstrate compassion, a non-judgmental attitude, and empathy towards persons served; ability to maintain a helping role and intervene in situations when appropriate
- Ability to work with children and families with multiple problems and an overall ability to relate and engage with children and families in the resolution of these problems
- Ability to facilitate and collaboratively work with the treatment team to establish and implement treatment plans, goals and behavioral interventions. Ability to develop outcomes measurements related to Person Centered Planning goals
- Ability to read, speak and write fluent English; strong oral and written communication skills including legible handwriting; ability to write simple and complex sentences with proper use of grammar and word order. Ability to work in team relationships and communicate professionally and extensively with government regulatory personnel, staff, service provides, clients and families
- Ability to be culturally competent and relate to multi-problem families
Additional Work Requirements
- Unrestricted, valid drivers license with good driving record. Reliable personal transportation suitable for safely transporting persons served; proof of current automobile insurance
