The primary emphasis of this position is to provide case management, therapy, crisis, and assessment services to adults with mental health, substance use disorder and homelessness-related issues in supported housing settings.Primary Duties and Responsibilities Provides individual behavioral health counseling and case management. Provides group therapy and rehabilitation designed to support residents. Coordinates with other providers to support residents in accessing primary medical care. Provide outreach to HomeRise residents who may otherwise not access services by traveling to their supportive housing locations and providing on-site services. Refers residents as needed for mental health and/or substance use disorder treatment. Conducts initial psychosocial assessments and develops individualized treatment plans and adjusts as needed. Documentation quality and timeliness must meet agency standards. Provide culturally competent services using harm reduction and trauma informed principles. Provide consultation to staff regarding best practices, symptom management, mental health conditions and crisis intervention strategies. Responds to mental health crises; provides immediate access to services, de-escalates, and follow-up care. Maintain a productivity target of 16 hours of services per week. Participate in agency and outside meetings, committees and workgroups. Performs rotating on-call and other duties as assigned.Minimum Qualifications California Licensed or License Eligible LCSW, LMFT, Psy.D., ASW, AMFT, LPCC, APCC Two years clinical experience working with adults with a history of homelessness or mental health/substance use disorder. Knowledgeable about San Francisco community resources that serve our population preferred. Strong practical and theoretical foundation in clinical MH/SUD evidence-based best practices. Must possess exceptional verbal and written communication skills. Proficiency in electronic medical record systems and Microsoft office platform. Willingness to work non-traditional hours as the job necessitates. Must maintain First Aid and CPR Certifications.
