**Schedule:** MondayFriday | 8:00 AM 5:00 PM EST

**Location:** Remote with 5075% field travel in Passaic County and surrounding areas

Are you a Licensed Social Worker with a passion for behavioral health and community-based care? Join our mission-driven team as a **Field** **Behavioral Health Case Manager** , where youll play a vital role in supporting members with complex needs through compassionate, coordinated care.

**What Youll Do:**

As a Behavioral Health Case Manager, you will:

+ Conduct in-home, face-to-face assessments for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Programs (D-SNP/FIDE).

+ Evaluate behavioral, physical, and psychosocial needs to develop individualized care plans.

+ Coordinate services such as adult/pediatric medical daycare, personal care assistance, and nursing facility placements.

+ Collaborate with members, families, providers, and interdisciplinary teams to ensure appropriate care and community resource access.

+ Advocate for members during transitions of care and attend interdisciplinary meetings.

+ Accurately document assessments and care plans in the electronic health record.

+ Mentor new hires once proficient in your role.

**What You Bring:**

**Required:**

+ Must reside in **Passaic County, NJ**

+ Active **Licensed Clinical Social Worker (LCSW)** or **Licensed Social Worker (LSW)** in New Jersey

+ **2+ years** of behavioral health experience

+ **1+ year** of experience using computers and Microsoft Office

+ Reliable transportation and willingness to travel **5075%** within Passaic County and surrounding areas

+ Successful completion of **NJ Choice Certification** (80%+ exam score, training modules, and field mentoring)

**Preferred:**

+ Experience in **case management** or **discharge planning**

+ Background in **Managed Care**

+ Strong **crisis intervention** skills

**Education:**

+ **Masters Degree** in Social Work or a Behavioral Health-related field (required)

**Pay Range**

The typical pay range for this role is:

$72,627.00 - $155,538.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
