Social Worker

A Skilled Nursing Facility in Camillus, NY is currently seeking a compassionate and dedicated Social Worker to join our team full-time.

Key Responsibilities:

• Support residents in maintaining or improving their ability to meet daily physical, emotional, and psychosocial needs

• Assess and identify each residents unique social and emotional needs

• Create and update personalized care plans based on ongoing assessments

• Collaborate on admission planning, discharge coordination, and transfers to other care settings

• Participate in interdisciplinary care plan meetings to address residents social and emotional well-being

Qualifications:

• LMSW preferred

• Experience in long-term care or sub-acute settings required
