Join an interdisciplinary team dedicated to compassionate, person-centered care. As a Case Manager , you'll provide direct therapeutic support, manage care plans, and coordinate services for individuals in need.

What You'll Do

Clinical Responsibilities:

  • Provide comprehensive psychosocial assessments.
  • Assure responsibility for the development & implementation of each assigned consumer's Individual Plan of Service when necessary.
  • Link consumers to necessary ancillary services.
  • Act as an advocate and/or a liaison coordinating services among providers of service and referral source.
  • Maintain consumer records - meeting all appropriate standards according to HNBHS and all regulatory bodies.
  • Participate in legal proceedings including petitioning and testifying in clinical emergencies.
  • Facilitate the coordination of consumer intakes, discharges, and transfers to and from other facilities programs, agencies.
  • Assist in linking transportation. Obtain relevant medical/treatment information from appointments.
  • Link and coordinate follow-up of professional recommendations.
  • Facilitate communication with guardians, payees, and family members regarding consumer care, financial and treatment issues.
  • Meet with assigned consumers face-to-face as deemed medically necessary in their Individual Plan of Service.
  • Provide after hours on-call services as designated by HNBHS.
  • Facilitate hospitalizations as needed and monitor progress by attending hospital medical/team meetings and coordinating with hospital social worker.
  • Provides consultation regarding clinical and program issues, direct care staff where applicable.
  • Participate in consumer program reviews and team meetings, where applicable.
  • Link consumers with community organization and community integration.

Administrative Responsibilities:

  • Participate in committees, task forces, and other related meetings and activities as assigned by supervisor.
  • Complete Annual Trainings
  • Practice according to licensing and accreditation for assigned program(s).
  • Attend agency meetings: clinical, administration and management.
  • Network with relevant agencies and providers as needed.
  • Follow general safety practices. Report any unsafe conditions to the manager of that location.
  • Complete and submit Service Activity Logs (SAL's) in accordance with agency policy/protocols.
  • Coordinate benefits and verify benefit status monthly.

Work Schedule & On-Call Compensation

Standard Hours: Monday-Friday, 9:00am-5:00pm, on-site

Flexible scheduling available based on caseload and program needs

On-Call Rotation: Participation required; schedule varies by location/program

  • One day per week every other week
  • One weekend every other month

On-Call Pay Enhancements

  • Weekdays (Mon-Fri): $50 per on-call shift
  • Weekends (Sat-Sun): $100 per on-call shift
  • Holidays: $150 per on-call shift

Qualifications

A Bachelor's degree in Social Work, Psychology , or Counseling is required . Candidates with a Bachelor's degree in a Human Services field may also qualify if they hold a valid Social Service Technician (SST) certificate.

Candidates must hold or be in the process of obtaining one of the following licenses:

  • Social Work: Licensed Bachelor Social Worker (LBSW) or Limited Licensed Bachelor Social Worker (LLBSW)
  • Psychology: Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP)
  • Counseling: Limited License Counselor (LLC) or Licensed Professional Counselor (LPC)

Candidates with a Master's degree in Social Work may also be considered, provided they are eligible for or in the process of obtaining one of the following credentials:

  • Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW)
