The ACT Case Manager provides an ongoing non-clinical assessment of all clients' individual recovery needs; serves as a team member responsible for establishing and monitoring client progress in meeting recovery goals; coordinates and provides linkage to services; acts as the liaison between client and community, building relationships with families and community agencies. The ACT Case Manager advocates on client's behalf for needed resources; provides ongoing assessment and encouragement in attending to and meeting Activities of Daily Living (ADLs) and provides services and information regarding housing, community resources and benefits. The ACT Case Manager is a positive team player that promotes a culture of inclusion and participation.
Required Education and Experience
Bachelor's degree in psychology, social work or other behavioral health related field
At least one year of full-time employment experience working directly with adults in a human service setting post degree. A Master's degree in psychology, social work or other behavioral health related field may be substituted for this experience
Must complete a case management or service coordinator certification program offered and approved by the state with six months of employment
Valid driver's license, reliable operating transportation, and proof of current auto insurance
Preferred Education and Experience
Master's degree preferred
Knowledge of mental health issues, community supports and ability to work well in a team environment
Knowledge of Recovery based interventions in Case Management services

Case Management/Rehabilitation/Psychosocial Care
Provide direct face-to-face services to promote socialization, meaningful daily activity and recovery
Provide psycho-education relating to health management, natural supports and individual strengths
Assist clients in development of their recovery plan, including psychiatric advance directives and crisis plan
Implement and coordinate recovery plans for individual clients as assigned, including evaluation, consultation, education, and follow-up services
Provide ongoing client health education, communication skills with healthcare personnel and medication monitoring
Provide advocacy services on behalf of clients
Collaboration and Communication
Serve as an active member of the client's recovery team, including participation in team meetings and treatment planning meetings to assist clients with identifying and attaining their recovery goals
Collaborate, coordinate, and provide resources for individual clients as an extension of the team services
Serve as a community liaison to link clients to services in the community and assist in building family, friend, and community relationships
Serve as liaison between clients and the recovery team
Inform supervisor, and other members of the recovery team as indicated, of issues/concerns related to individual clients
