Job Description The Case Manager is responsible for building strong communication and relationships with clients and their collateral partners, including schools, funding partners, therapists, agencies, clinicians, supervisors, and foster homes. This role involves setting limits for youth, developing interventions, and setting goals for individual success in collaboration with the client's guardians, therapists, and other providers. The Case Manager will initiate and monitor external recommendation processes and treatment planning for their caseload, ensuring compliance with policies and procedures, including all contract sources. They will also interact effectively and report timely for computer reports, tracking, and paper reports according to agency protocols. As needed, participation in on-call services is required. Furthermore, the Case Manager will represent the agency on task forces and community boards regarding interests and possess strong interpersonal skills throughout all levels of the agency. Attendance at meetings across the agency and with collateral partners is expected, along with staying informed about trends and significant events in the field of Human Services and Case Management.
