JOB DESCRIPTION:

Duties and Responsibilities: Provide care management services for children, adolescents, adults, and their families who have experienced or witnessed trauma.
Duties Include:

• Coordinate Referrals/linkages to meet individual or youth/family needs (e.g., social services, primary care, juvenile justice services, criminal justice system, employment) and provide follow-up services.
• Develop, maintain, and utilize community referral sources.
• Conduct project eligibility and co-occurring disorder screening.
• Provide referrals for ACA enrollment.
• Ensure that identified individuals receive needed assistance and support services.
• Track/support client treatment plan progress, trauma/risk status monitoring, and follow-up.
• Provide trauma-related education and facilitate preventative care.
• Provide immediate assistance to clients/families in crisis.
• Assist veterans/military families in navigating web portals for behavioral health and treatment support; and
• Attend all meetings, maintain records, and coordinate/consult with community organizations.
• Complete intake needs assessment and treatment planning.
• Attend multi-disciplinary team or community at risk meetings on behalf of clients.

Qualifications: Bachelors degree in social work, psychology, or related field with at least two years experience in trauma. Bilingual preferred.
Skills and Knowledge Required: Trained in individual trauma issues; experience in child/adolescent trauma preferred. Knowledge of the focus population, able to create a culture of respect, commitment to project goals.
Personal Qualities: Must value cultural differences, work independently and/or as a member of a team, problem solve, and be genuinely interested in working with individuals who have experienced and are impacted by trauma.
