What you will do:
The Lead Social Work Position reports to the Manager of Social Work. The specific duties include:
Recruiting, interviewing, hiring, and training staff and volunteers. Continually reviews of Social Work's processes for the effective flow of patients through the discharge planning.
Oversight of the complex social cases with creation of a plan of care and placement in the community efficiently and effectively.
Development of educational materials for new orientees and creation of orientation plans for each individually newly entering the department.
Establishing policies and procedures.
Collecting data and other information to evaluate program impact. Overseeing performance improvement projects to realize
process improvement achieving targeted outcomes.
Oversight of educational programming and supervision of staff seeking LCSW.
On the basis of preliminary screening assesses psychosocial risk, appropriateness and adequacy of support, reaction to illness and ability to cope-creates strategies to move the family forward with expectations for participation.
Intervenes with patients and families regarding emotional, social, financial consequences of illness and disability.
Provides intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse and assault.
Serves as a resource person and provides counseling and interventions related to treatment decisions and end of life issues.
Advocate for patient and family empowerment and independence to make autonomous health care decisions and access to needed services within the healthcare system.
What you'll need:
Masters Degree in Social Work or related field. Current LSW license in good standing. LCSW preferred.
Three to five (3-5) years inpatient experience required. Two (2) years supervisory experience preferred.
Ability to effectively interact with all levels of hospital, governmental and medical staff.
Excellent written and verbal communication skills
Compensation:
Salary Range: $29.78 - 48.84 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
