Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.

Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Job Title: Co-Response Care Coordinator (Nashville, TN)

JOB SUMMARY: Provide face-to-face and telephone follow up and resource coordination to consumers serviced by the REACH/PIC Programs. Provide mental health support and care coordination while completing follow-ups and co-responding with the Partners in Care/REACH Programs

RESPONSIBILITIES:

  • Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
  • Ensure consumers are seen upon discharge from the hospital and Diversion Services within Agency standards.
  • Ensure timely and appropriate documentation as required by the Agency.
  • Develop and maintain community relations through prompt follow through with contacts
  • Ensure efficient transition of consumers when needed within the Agency and the community.
  • Respond to urgent/crisis consumer needs as with coordination with Co-Response supervisors, Co-Response counselors, and Co-Response program managers
  • Able to reliably work independently with minimum structure and supervision.
  • Provide on-going outreach and in-reach to individuals encountered and identified by Co-Response team in the community
  • Advocate for consumers needs to be met.
  • Refer consumers to appropriate services
  • Follow-up with consumers and other agencies to ensure consumer needs have been addressed.
  • Re-engage or re-establish contact with consumers who have not followed through on necessary services
  • Develop rapport with individuals identified by Co-Response team to assess and assist with their needs.
  • Research and be familiar with available resources and share resources with appropriate Co-Response staff.
  • Develop and maintain relationships with other agency staff to facilitate teamwork among the community.
  • Develop and maintain community and MHC inter-departmental relations including prompt follow-up with all contacts.
  • Coordinate Transportation for consumers as needed to assist in linking to resources.
  • Participate in community coordinated events, campaigns, and initiatives as deemed appropriate by Program Manager.
  • Collaborate with other agencies.
  • Able to approach and effectively work with individuals in all types of settings including individuals on the streets, in encampments, shelters, apartments, and homes.
  • Facilitate trainings and information sessions for community partners

OTHER RESPONSIBILITIES:

  • Comply with Metro Nashville Fire department and Metro Nashville Police Department (MNPD) policies pertaining to professional conduct and reporting close personal relationships to MHC department administration.

REQUIREMENTS:

  • A mental health professional with specialized training in crisis intervention with a minimum of a Bachelors degree in a related field (counseling, psychology or social work)
