Work Setting: Hybrid (community-based with field visits to homes and facilities)
Compensation: $38.00 $43.00 per hour (based on experience)
Travel Required: Yes (valid drivers license and reliable, insured transportation required)

Position Summary
The Medical Social Worker Hospice is an essential member of the interdisciplinary care team, providing emotional, psychosocial, and practical support to patients facing terminal illness and their families. You will guide individuals through complex medical decisions, support them in times of emotional crisis, and ensure that end-of-life care honors their values and needs.
Key Responsibilities
Direct Patient & Family SupportProvide one-on-one and group counseling for patients and caregivers coping with terminal illness, loss, and grief
Support families in navigating emotional, psychological, and social challenges during hospice care
Educate patients and families about available support systems, both clinical and community-based
Collaborate with hospice team members to develop holistic care plans aligned with patient goals and family dynamics
Facilitate advance care planning discussions (e.g., advance directives, comfort care preferences)
Act as a liaison to ensure culturally sensitive, patient-directed care
Provide immediate support during emotional or psychosocial crises
Connect families to services including financial aid, respite care, and legal assistance
Help families access healthcare navigation resources
Provide pre- and post-death grief counseling to families
Conduct bereavement follow-ups and offer referrals to grief support services
Assist with family transitions and emotional healing
Maintain timely and thorough records of assessments, care plans, interventions, and outcomes
Ensure all actions align with organizational policy and regulatory compliance standards
Required Qualifications

Masters Degree in Social Work (MSW) from a CSWE-accredited program
Active LCSW license (California)
At least 1 year of healthcare experience (hospice or palliative care preferred)
Strong clinical skills in end-of-life counseling, resource navigation, and crisis management
Current CPR certification
Excellent communication and documentation abilities
Valid drivers license and reliable, insured vehicle
Preferred Qualifications

Knowledge of cultural, ethical, and spiritual dynamics surrounding end-of-life care
Familiarity with EMR systems and hospice documentation practices
