Make a difference in the community with our Venice Family Clinic as a Housing Case Manager. With guidance and supervision from the Case Management Supervisor, the Housing Case Manager provides high quality case management to medically complex patients experiencing homelessness. This position provides direct housing navigation and stability support in both field-based and clinic settings for patients eligible for the Community Supports housing program.

Salary range: $30.53 - $40.30 Hourly

Required:
  • Minimum of one year experience in homeless service case management.
  • Knowledge of community resources, housing first methods, ICMS, tenant advocacy, harm reduction, trauma informed care, motivational interviewing and general clinical knowledge.
  • Must maintain a valid California Drivers License and auto insurance in good standing; ability to operate both personal and company owned vehicles to travel between worksites and other locations as required.
  • Ability to create and maintain boundaries with clients
  • Ability to follow through on assignments, complete required program documentation, and skill in managing multiple deadlines and demands.
  • Ability to work independently and as part of a group, and skill in maintaining cooperative working relationships.
  • Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality.
  • Ability to work effectively with people of diverse cultural and educational backgrounds.
  • Skill in creating flyers and other program-promoting documents.
Position is subject to the California DMV's "Pull Notice System" and continued employment is contingent upon proof of a satisfactory driving record.
