Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each clients specific needs and goals. Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

JOB SUMMARY:
Prototypes Outpatient mental health services provides comprehensive mental health services for our
community ages birth through older age. Our continuum of care ranges from prevention and early
intervention to intensive outpatient services for those with more serious and chronic conditions, including
Full-Service Partnership (FSP) services. Staff provide services at our Prototypes sites and in the community.
These services can include individual and group services, medication support, case management, and
coordination with other healthcare services.
The Case Manager provides service to clients through coordinating care across the HealthRIGHT 360s
continuum of care, and with collaborating human services providers, as well as collaborating with clients
to complete intake documents, outcome measures, and benefit applications. The Case Manager I works
in collaboration with other members of the multidisciplinary team to ensure best possible treatment.

The Case Manager maintains a caseload and provides care by linking the client with appropriate services
to address specific needs such as: benefits, mental health, physical health, employment, probation, DCFS,
employment, housing, community resources, outpatient substance use disorder services, and aftercare.
The Case Manager I offers support services to clients to increase self-efficacy, self-advocacy, basic life
skills, coping strategies, self-management of biopsychosocial needs, benefits and resources, and
reintegration into the community. The Case Manager I works in collaboration with other members of the
multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in
accordance with HealthRIGHT 360s philosophy, goals, policies, mission, and vision. Services may be
provided in outpatient office, residential treatment, or the field, based on client and program need.
KEY RESPONSIBILITIES
- Provides intake services to clients, including opening the treatment episode, communicating as
needed with EHR and Billing teams, entering financial/benefit information, and collaborating with
program supervisors. - Supports the client in applying for Medi-Cal or transferring Medi-Cal to LA County when
appropriate. - Gathers information on clients healthcare providers and completes all releases of information.
- Coordinates communication and external service linkage in collaboration with program staff.
- Serves as point of contact for clients receiving medication-only or Wellness services and ensures
client records are kept updated. - Collaborates with clients to complete the WHO-QOL, OMAs, KECs, and other outcome measures
as assigned. - Completes DMH NET assessment to identify CM needs for the Problem List and adds to Problem
List as appropriate. - Provides services in individual, family, and group formats.
- Seeks consultation throughout the treatment episode, especially when encountering new and/or
high-risk clinical circumstances. - Completes client UA tests and documents test results (when assigned).
- Maintains frequent communication with the treatment team and engages in regular consults,
clinical team meetings, and case conferences. - Delivers services in outpatient, residential, or field depending on assignments and/or client
need(s), including before and after business hours. - Meets expected client care hours (productivity expectation is 25 client care hours per week/ 1500
minutes weekly). - Provide translation services as needed.
- Provide support for parents and caregivers to navigate health systems and benefits.
- Provide field-based services as required by program
- Collaborate with program staff to offer support with OMAs, KECs, and other outcome measures
as assigned.
- Complete and submit rent invoices/ flex fund request/ and vendor check requests as needed.
- Complete KEC/OMAs at scheduled due date.
- Participate in Outreach and Engagement Process (including commuting into the field as needed)
to support client intakes.
- Outreach and engagement to assist with intakes for referred individuals and clients struggling to
meet probation mandates (Including commuting into the field as needed). - Assist with completion of weekly outreach and engagement reports.
- Provide jail in-reach and post-release care coordination as needed. (Services may take place in
the office, jails, and probation HUBs) - Coordinate with program and clients to obtain needed community resources.
- Collaborate with outside agencies to help clients obtain needed community resources (housing,
sober livings, substance abuse recovery agencies, medical facilities, employment/education
agencies, DPSS, DMV or other needed resources) - Write and complete all progress notes within 72 hours of service delivery.
- Maintains compliance with due dates for all clinical documentation.
- Writes client progress letters and court reports and obtains final approval from the supervisor.
- Documentation must maintain in compliance with agency policy and procedures, HIPAA, 42-CFR,
DMH, and SAPC standards. - Completes all assigned URCs within a timely matter.
QUALIFICATIONS:
Education, Certification, and Experience
Required:
- High school diploma or equivalent.
- Minimum of one year experience in the human service field and/or demonstrated expertise in
areas such as chronic mental health issues, substance use, family treatment, and recovery. - Knowledge of and experience with providing trauma informed services.
- Valid drivers license with proof of insurance.
Desired:
- Experience delivering evidence-based practices.
- Two years experience in the human service field and/or demonstrated expertise in substance
abuse treatment, relapse prevention, and recovery.
Background Clearance:
- Must not be on active parole or probation.
- Must complete a background check and live scan.
- Must be fully vaccinated against COVID-19, including booster shots. Medical and religious
exemptions available.
