Job Description

South Shore Support Services, and assists families, children, youth and adult family members referred by the Department of Developmental Services (DDS) in developing annual Family Support Plans and assist in accessing resources needed to obtain goals. The purpose of the South Shore Support Services is to:

  • Promote ability of families referred by the Department of Development Services to support their members in strengthening life skills, peer connections, parent connections and self-care, and positive family life;
  • Promote disabled individuals in achieving life skills, education and employment goals;
  • Develop and utilize resources to support families caring for disabled members;
  • Provide a range of services, including case management, assessment, service planning, referrals, crisis intervention and guidance in using available resources to assist families in meeting service goals.

The Family Support Case Manager is supervised by the Director of Family Support, and works directly with families, children and youth in their homes and community.
